Thank you for reaching out and wanting to start this journey of becoming a licensed foster parent. The first step to getting this ball rolling is to start the background check process. Please follow the steps below.

BCI PROCESS

 
All potential foster parents must complete a background check by following these steps.

 

• Complete the BCI form
  

 ( Select the FOSTER CARE application form & be sure to carefully read each question, if answering YES to questions #2, #3 or #4 additional documentation is required. Be sure to record FOUNDATIONS as the

'Program Name' on the  bottom of Page 1 )

• Call our Office Manager Brodi Ika 801 792 4867 to have her complete Section 9 on this form before calling to book a Live Scan appt.

• Call your nearest LIVE SCAN office to make an appointment to have your finger prints taken
  

•  Once the live scan appointment is complete, bring the BCI form and a copy of your current drivers license and social security card back to the Foundations Office where we will complete the process and mail the form to the Dept of Human Services.
  

•  The process can take up to 2 - 4 weeks before getting the approval returned.
